You will need to complete and send us your enrollment
form to get your postpartum visit arranged. Either

form (PDF download is below) or call us at 303-447-7174
to have us send a form in the mail.
Hospital Discharge
After receiving your enrollment form, a Beyond Birth midwife
will contact you to confirm your visit. You should
then download the hospital form (PDF download
below) to take to the hospital when you go to
deliver your baby. This form needs to be completed
with delivery and infant information before
you are discharged home.
Insurance Reimbursement
If you are interested in having insurance reimbursement
for your Beyond Birth visit(s), we recommend you contact
your obstetric and pediatric providers for orders
for our services and give them a physician's order
form (PDF download below.) Most Boulder offices
will be familiar with Beyond Birth and will be happy to
give you the appropriate order(s) for insurance
reimbursement purposes.
Meet the Midwife Evenings
Every month there is an opportunity to meet the
Beyond Birth midwives at our “Meet the Midwife”
evenings. These evenings are
an informal way to meet the staff and have some
questions answered. See the "Meet
the Midwife" section for planned dates
and how to RSVP.
Beyond Birth Midwifery Care,
LLC is a Limited Liability Company providing postpartum
home visits by Certified Nurse Midwives. Our visits,
which offer a professional assessment of both the new
mom and the new baby, are done on a fee for service
basis. We will generate a superbill with appropriate
coding which can be submitted to your insurance company.
Reimbursement will vary depending
on your benefits, so check with your own carrier. A
referral from both your obstetric and pediatric providers
will be necessary in order to have both visits reimbursed;
but a referral is not necessary in order to take advantage
of our services.

Comprehensive Exams:
For both mother and baby = $180
For mom only = $100
For baby only = $100
Revisit exams are prorated
at $180/hour in 20 minute increments.
Prices include travel to
homes inside Boulder city limits. Mileage reimbursement
for locations outside the city of Boulder will be billed
at $0.43/mile.
